Job Description: Consultant, Advisory Board, Trauma Research Foundation

Janina Fisher, Ph.D. is a licensed Clinical Psychologist and Instructor at the Trauma Center, an outpatient clinic and research center founded by Bessel van der Kolk. Known for her expertise as both a therapist and consultant, she is also a past president of the New England Society for the Treatment of Trauma and Dissociation, an EMDR International Association Credit Provider, a faculty member of the Sensorimotor Psychotherapy Institute, and a former Instructor, Harvard Medical School. Dr. Fisher has been an invited speaker at the Cape Cod Institute, Harvard Medical School Conference Series, the EMDR International Association Annual Conference, University of Wisconsin, University of Westminster in London, the Psychotraumatology Institute of Europe, and the Esalen Institute. Dr. Fisher lectures and teaches nationally and internationally on topics related to the integration of research and treatment and how to introduce these newer trauma treatment paradigms in traditional therapeutic approaches.

With TIST Level 1, professionals learned the concepts of the TIST model and how to apply it. When creating TIST Level 2, professionals indicated a need for applicable skills and techniques as well as more clarity on using this technique with highly traumatized clients. TIST Level 3 is designed to give advanced practitioner skills, so that participants can bring the TIST method into the real world with confidence and poise.

Overall

The TIST Certification Training is designed for attainment of advanced practitioner skills in the TIST method.  Trauma-Informed Stabilization Treatment or TIST is a mindfulness-based, trauma-informed treatment approach that integrates concepts and techniques from well-established models, such as Internal Family Systems, Sensorimotor Psychotherapy and clinical hypnosis ego state therapy.

Module 1 – TIST Certification:  What does it mean to be a TIST-certified therapist?

• Identify four manifestations of trauma-related parts in the client’s narrative
• Describe the structural dissociation model as a trauma model
• Articulate three techniques for helping clients notice instead of react to triggers

Module 2 – Case Conceptualization and Treatment Goals

• Discuss how to de-construct symptoms as communications from trauma-related parts
• Formulate four parts-focused treatment goals
• Identify three signs of trauma-related disorganized attachment

Module 3 – Foundational Skills

• Describe three ways of increasing client ability to mindfully notice distressing emotions and impulses
• Demonstrate ability to use parts language with clients
• Identify three ways of recognizing ‘blending’ with a part

Module 4 – Healing the Legacy of the Past

• Identify three ‘missing’ experiences of trauma-related parts
• Demonstrate the ability to facilitate increased client compassion for the parts
• Describe three strategies for helping clients develop earned secure attachment

The TIST Certification Training is designed for attainment of advanced practitioner skills in the TIST method.   It consists of four live modules providing an opportunity for questions and discussion and six practice sessions in small groups supervised by a facilitator.  Participants will also be asked to present a written case presentation formulating a case from a TIST perspective.

MODULE ONE: TIST Certification:  What does it mean to be a TIST-certified therapist?

Certification represents attainment of the building blocks of the model.  In this module, we will discuss the basic foundational skills needed to acquire competence in the method.

• How to “hear” the voices of the parts in the client’s narrative
• How to conceptualize a case from a TIST perspective
• How to introduce the model to a client
• How to help clients replace reactivity with mindful noticing of parts
• How to help clients ‘change the story’ they tell themselves

MODULE TWO: Case Conceptualization and Treatment Goals

• Case conceptualization and formulation of treatment goals are necessary skills to determine if TIST is appropriate for a given client. Students will present cases and identify the issues and symptoms that require TIST for treatment. They will also demonstrate the ability to describe treatment goals consistent with the model.
• Diagnoses and symptoms most commonly indicative of the need for TIST
• Mastering the ability to de-construct symptoms and issues as manifestations of parts
• Using the Structural Dissociation model to understand the dynamics of unsafe and addictive behavior
• Formulating parts-focused treatment goals
• Framing the work to be done with the client: a collaborative approach to treatment goals
• Anticipating trauma-related treatment challenges: ‘pull’ of Attach parts, anger and de-valuing, flights from treatment, hopelessness

MODULE THREE: Foundational Skills

• “Foundational skills” are the prerequisite for success in treatment. Attainment of these basic skills is the focus of this module, including the ability to:
• Mindfully notice and name distressing emotions and impulses as parts
• Implement the ‘language of parts’ rather than using the language of ‘I’
• Help clients to recognize blending as information about a part
• Teach clients how to unblend from the feelings and impulses of parts
• Help clients increase their access to “C” qualities
• Help clients engage in internal dialogue with their parts
• Foster understanding of and empathy for the parts at the client’s pace

MODULE FOUR: Healing the Legacy of the Past

• Moving from internal conversation to internal healing experiences takes therapist skill. The focus of this module is on building the client’s ability to offer reparative experiences to trauma-related parts, including
• Developing the ability to sense the ‘missing’ experiences of wounded parts
• Supporting increased compassion for all parts
• Negotiating protector parts’ blockage of compassion and empathy
• Facilitating clients’ motivation to protect or care for the parts
• Capitalizing on moments of compassion and attunement to provide reparative experiences for a part
• Developing strategies to maintain caring relationships to parts
